Sosteri, the health district of Savonlinna region in Eastern Finland, has qualified Synesa Solutions Ltd as its partner through a public procurement process. Synesa Solutions and Sosteri are working together in a project for population health. The project is included in the Development programme for health and social services centres of the future. The programme is funded by the Ministry of Social Affairs and Health in Finland.
The project targets to improve the care compliance of type 2 diabetes patients in the region. Sosteri has already modelled its target care paths and service model related to diabetes treatment. The new population health pilot project models the actual treatment pathways, compares them with target treatment pathways, and analyzes the achieved health outcomes. In the project, the health outcomes are linked to process analytics in an innovative way, analyzing the output of the process through data modeling and process mining. Here, the eDromos software of Synesa Solutions Ltd is fully utilized. The dashboards for continuous care monitoring are also designed in the project.
Sosteri desires to better understand the customer segmentation of patients with diagnosed diabetes as well as the success of the related care paths. It is also Sosteri’s interest to better understand the health outcomes in each segment. The eDromos software and its ability to easily build simulation-based future scenarios help achieve these goals. eDromos enables simulating different new methods of care as well as assessing the impact of population changes in the future.
Pia Karttunen, the project manager in Sosteri, describes the project:
”The project in population health is aiming to improve the quality and health outcomes of primary care as well as the total health of the population. Sosteri has been the leader in the development of care paths and processes since 2016. Now we are focusing on the improvement of care for diabetes and aiming to find ways to further develop the care for different customer segments and their needs.”
Toni Ruohonen, CEO of Synesa, describes the new partnership:
”The project in population health with Sosteri is extremely interesting since it is focusing on the care of type II diabetes, which is the greatest health issue in all modern societies. The potential for improvements and their value for society is impressive. The project utilizes all the sophisticated and innovative features of the eDromos software covering data mining, process and care path modeling, simulation, and dashboards to support the care personnel in the future.”
Sosteri is the health district of Eastern Finland providing health and social services for the population in Savonlinna, Rantasalmi, Enonkoski, and Sulkava.
